High Quality, Physician Driven Care

The Foundation for Medical Care (FMC) for Mendocino and Lake Counties is one of fifteen such county wide Foundations in the state of California with over 350 member physicians, ancillary providers, hospitals, and clinics.

With a membership exclusive to physicians, the Foundation is truly a physician-run organization. Member physicians elect a fifteen members Board of Trustees to set policy and provider guidance for staff activities.

One of the ways in which the Foundation pursues its stated purpose is by sponsoring a Preferred Provider Organization (PPO). The PPO is a network of participating hospitals, physicians, and ancillary providers who have agreed to accept discounted fees and agree to physician peer review and utilization management as a means to maintain quality care. The PPO emphasizes patient choice, allowing access to a broad selection of providers who agree to deliver care in a cost-effective manner. As a Foundation member, physicians are automatically PPO providers as well.

The Foundation PPO is part of a statewide Foundation sponsored PPO network through our affiliation with the California Foundation for Medical Care (CFMC).

CFMC's network has over 300 hospitals, 34,000 physicians, and 5,500 ancillary providers, as well as ambulatory care centers, diagnostic imaging centers, mental health and chemical dependency centers, rehabilitation and physician therapy centers, surgicenters, laboratories, and more.

Blue Shield of California Health claims and employer group's Self-Funded Dental and Vision claims are also processed at the Mendocino and Lake Counties FMC and provides a Customer Service department to provide prompt attention to our members.

Our Customer Service department is open 9am-Noon & 1-5 pm PT, Monday - Friday.
